Thank you for choosing West Michigan Orthopaedics as your healthcare provider! To help ensure that your experience is both pleasant and valuable, we have established the following guidelines:

• Be prepared with the history and description of your problem when you come to your appointment.
• Please wear comfortable clothing that will allow your physician to easily examine the symptomatic area.
• Please bring with you any x-rays that have been taken within the last six months pertaining to your orthopaedic condition.
• Please complete our new patient forms and bring them with you to your appointment. If you have not already received these forms, they are available for download here.
• Please bring your insurance card(s) with you.
• Obtain a referral from your primary care physician prior to your appointment if required by your insurance company.
• If the patient is a minor, a parent or legal guardian must accompany him or her to the initial office visit.
